Heart failure due to hypocalcemia is a strange presentation of metabolic heart disease, reversible in most cases. We present the case of a woman of 23-years-old, diagnosed of severe heart failure. During his hospitalization, there was a permanent prolongation in the corrected Q-T interval, associated with hypocalcemia secondary to recently diagnosed hypoparathyroidism. To prevent the risk of sudden death, an implantable cardioverter-defibrillator was placed; however, after 6 months of treatment, there wasn`t an imporved of the ejection fraction, which determines a poor prognosis of the disease
