Congenital Lobar Emphysema in a newborn. Case report

Introduction: Congenital Lobar Emphysema (CLE) is an infrequent respiratory malformation that occurs with hyperinflation and irreversible septal destruction, it is associated with a partial bronchial obstruction; there are cases where no airway alterations have been found.
Clinical case: It is a case of a newborn, male, with no relevant history, at 5 minutes of life the baby presents non-reversible respiratory distress, being transferred to neonatology. The evolution is torpid with mechanical ventilation for 40 days, without the possibility of extubation. The pediatric surgery service raised suspicion of CLE, planning a thoracotomy and revealing a non-expandable emphysematous right middle lobe that was finally resected. Due to a favorable evolution, discharge is raised at 57 days of life.
Conclusion: CLE is a rare entity, it represents a challenge for pediatric surgeons and neonatologists; whose diagnosis and early surgical treatment determine a better prognosis.
