Abstract

Introduction: The Neutrophil-to-Lymphocyte Ratio (NLR) is a simple and inexpensive marker of subclinical inflammation, which is easily calculated from differential white blood cell counts. NLR provides information on immune and inflammatory pathways that may make it a potential marker to predict Acute Appendicitis and its severity.

Objective: To determine the usefulness of NLR as a marker of Acute Complicated Appendicitis in the Macará Basic Hospital during 2018-2021.

Materials and methods: An observational analytical retrospective study was carried out, with a universe of 196 patients who attended the emergency service with a diagnosis of Acute Appendicitis who met the inclusion criteria, at the Basic Hospital of Macará, during the period 2018-2021. An analysis of the statistical variables (age, gender, Acute Appendicitis, complicated Acute Appendicitis, Neutrophil to Lymphocyte Ratio) was performed in the EPIINFO 7 program. PR (95% CI) and chi square (p <0.05) were used for statistical significance.

Results: The prevalence of complicated Acute Appendicitis was 37%. In the uncomplicated Acute Appendicitis group, the mean NLR was 4.93 (95% CI 4.34-5.52), while in the complicated Acute Appendicitis group the mean NLR was 13.32 (95% CI 11, 1 – 15.5). NLR > 8.8 was found to be a 2.37 times greater risk factor for complicated Acute Appendicitis, with a sensitivity of 58% and a specificity of 91%.

Conclusions: Neutrophil to Lymphocyte ratio is a good predictor of complicated Acute Appendicitis at the Macará Basic Hospital.
