The talon cusp or claw tooth is a variant of dens eva-inatus, this is an anatomical alteration that pro-jects as an additional cusp, commonly associated with the cingulum of the anterior teeth. It occurs in temporary and permanent dentition, varies in size, shape, length and way in which it is attached to the crown, it can be located on the palatine or vestibular, however it is mostly related to the permanent denti-tion in the maxilla and on the palatal surface. of the maxillary lateral incisors. Its etiology is not clear, it is usually associated with genetic factors and some pa-thologies. Radiological and topographical examina-tions are decisive to know the amount of pulp tissue inside the tubercle. The treatment is indicated in cases where there is occlusal interference, aesthetic compromise, caries, periodontal problems, irritation of the tongue during speech and chewing. Presentation of the case: this clinical case is unusual, it is a 21-year-old patient who comes with pain after a trauma in an upper left central incisor, intraoral ex-ploration and radiographic tests, the diagnosis was Cuspid Heel; a root canal treatment was carried out due to the disturbance and later the grinding of the protuberance. Discussion: dental development is a complex pro-cess and any alteration can be a factor in the deve-lopment of morphological anomalies such as Dens Evaginatus or Heel Cusp. The resolution of this case is an example of the variations of the treatment being dependent on the symptomatology, severity of the case and apical maturation.Key words: dental anomaly, dens evaginatus, talon cusp, claw cusp.
